If your health care provider has recommended that you be tested using the MaterniT21® PLUS test, the information below will act as a guide to help you understand your options in covering the cost of this test.

For uniformed service members and their families, your payment is determined by your TRICARE costshare agreement. Please call your TRICARE office for more information.
Insurance (HMO and PPO)
We will bill your insurance company directly for tests we perform within the United States. We do require:
- Completed test requisition form (TRF) with the “Option 1. Patient Insurance Billing Information” box selected.
- Complete insurance information, including a copy of your insurance card (front and back).
Explanation of Benefits (EOB)
The Explanation of Benefits (EOB form) is a statement sent by your health insurance company to you explaining what medical treatments and/or services were paid for on your behalf. You should review the EOB statement, but do not pay the amount listed. It is simply an explanation - not a bill.
Patient Assistance Program
We believe all patients should have access to the accurate tests we provide, regardless of financial circumstances. Available programs (subject to certain limitations) include prompt pay discounts, interest-free payment terms, and programs for patients with specific financial needs.
